Description Conduct comprehensive evaluations for patients in both inpatient and outpatient environments, crafting personalized treatment plans. Administer targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function. Work collaboratively with nurses, physicians, case managers, and a team of rehabilitation professionals to optimize patient outcomes. Educate patients and families on therapeutic goals, discharge planning solutions, and home exercise regimens. Maintain accurate documentation of evaluations, treatments, and patient progress as per facility and regulatory requirements. Engage in interdisciplinary team rounds and conferences to ensure coordinated and holistic care. Education

Doctorate in Physical Therapy (DPT) or a comparable degree from a recognized program. Certifications

State licensure as a

Physical Therapist . CPR certification preferred. Specialized certifications, such as in neurological or geriatric care, are beneficial but not mandatory. Skills

Prior experience is greatly preferred. Capability to treat patients with intricate medical and mobility challenges. Strong communication and teamwork abilities. Familiarity with digital charting and clinical documentation systems. Benefits
