Are you an experienced RN with a strong understanding of MDS and care planning? Napa Post Acute is seeking a detail-oriented
RN MDS Coordinator
to lead our assessment and care planning processes with accuracy, compassion, and clinical excellence.
What You'll Do: Coordinate and oversee the MDS assessment process to ensure timely and accurate submissions Develop and update individualized care plans with the interdisciplinary team Ensure compliance with all federal and state regulations, including PDPM guidelines Collaborate with clinical and administrative staff to optimize resident outcomes and reimbursement Provide staff education related to MDS processes and documentation What We're Looking For:
Active
Registered Nurse (RN)
license in California Minimum
1 year of MDS experience
in a skilled nursing facility Strong knowledge of
PDPM, CMS regulations, and RAI process Excellent organizational, communication, and critical thinking skills Passion for delivering quality care to seniors Why Join Us:
