Physical Therapist (PT)
to help our residents regain strength, restore independence, and live life to the fullest in our welcoming long-term care and rehabilitation community.
What You'll Do:
Provide skilled physical therapy evaluations, treatments, and care planning for residents. Collaborate with an interdisciplinary team (nursing, occupational therapy, speech therapy, physicians) to create and deliver individualized treatment plans. Track resident progress and adjust interventions to achieve optimal outcomes. Educate residents, families, and staff on safe mobility, functional independence, and injury prevention. Promote dignity, motivation, and confidence in every therapy session. What We're Looking For:
Current PA licensure as a Physical Therapist (required). Strong clinical skills with a heart for working with seniors in a post-acute or long-term care setting.
