Description Conduct thorough patient evaluations in both inpatient and outpatient environments and create tailored treatment plans Implement therapy interventions aimed at improving mobility, strength, balance, and overall functional ability Collaborate with a multidisciplinary team of nurses, physicians, case managers, and rehabilitation specialists to ensure optimal patient outcomes Guide patients and their families with insights on therapeutic goals, discharge planning, and home-based exercises Maintain comprehensive documentation of evaluations, treatments, and patient progress within compliance parameters Engage in team rounds and interdisciplinary meetings to foster a holistic approach to patient care Education

Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized program Certifications

State licensure as a

Physical Therapist CPR certification desired Additional specialized certifications (neurological, geriatric, etc.) are beneficial but not mandatory Skills

Preferred experience in a clinical setting Capability to manage patients with detailed medical and mobility needs Exceptional communication and teamwork skills Proficient in using digital documentation and clinical software Benefits
