Description Perform evaluations for patients in inpatient or outpatient environments, crafting tailored treatment plans Deliver physical therapy interventions focused on mobility, strength, balance, and functionality Collaborate diligently with nurses, physicians, case managers, and other rehab professionals to ensure optimal outcomes Educate patients and their families regarding therapeutic aims, discharge planning, and exercises for home Accurately document evaluations, treatment plans, and progress records consistent with facility and regulatory standards Engage in care team rounds and interdisciplinary meetings to guarantee synchronized care Education

Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited institution Certifications

State licensure as a

Physical Therapist Preferred CPR certification Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not compulsory Skills

Prior experience in a clinical setting is strongly preferred Proficient in treating patients with complex medical and mobility challenges Exceptional communication and teamwork abilities Familiarity with digital charting and clinical documentation systems Benefits
