Position Overview:
Location: Albuquerque, New Mexico
Patient Volume: patients per day
Schedule: Flexible Monday Friday, part-time or full-time options available (2 4 days/week)
Setting: Inpatient post-acute care
Key Responsibilities:
Diagnose medical conditions and manage chronic disease
Develop and implement individualized care plans
Build strong relationships with patients, families, and facility staff
Serve as a medical director for assigned facilities
Provide guidance, mentorship, and oversight to clinical staff
Conduct case reviews and promote high-quality care delivery
Collaborate with leadership on care coordination and quality initiatives
Represent your organization in the broader post-acute care community
What We re Looking For:
Clinicians with strong clinical and critical thinking skills
Excellent communication and collaboration abilities
A compassionate, patient-centered approach to senior care
Experience in post-acute care or leadership is a plus (not required training provided)
Active and unrestricted New Mexico medical license
Active and unrestricted DEA license (New Mexico)
