HIGHLIGHTS:
- $15,000 COMPLETION BONUS
- 24 months minimum of current Cardiac Cath Lab Technologist experience -REQUIRED
- Must have ARRT and AZ Radiologic Technologist license
- CAN APPLY FOR AZ LICENSE UPON OFFER
- No License Hits -REQUIRED
- Travel experience-Preferred
- OnCall: On call schedule to be determined by facility
- Weekend Description: Weekend schedule to be determined by facility
- Certifications: ARRT, BLS (AHA)
- State Licenses: Arizona
- Banner will not approve RTO within the first week or holidays. Can not exceed 7 days
ADDITIONAL DETAILS:
- Candidates with an AZ home address on their drivers license will not be considered-please include a copy in submission
- Certifications must be American Heart Association OR American Red Cross, active upon submission and INCLUDED IN SUBMISSION-REQUIRED
- PROFILE REQUIREMENTS:

- Resume with all facilities, facility size, beds, trauma level, and units listed
- Skills Checklist specific to the discipline (must be less than a year old)
- Active License (verification must be less than 30 days old)
- Certifications (must be active at time of submission)

- This is a Special Procedures Cath Lab Tech Position
- Banner has a specific cover page
- PLEASE vet all locations to the travelers and add all experienced departments candidates are willing to ACCEPT/work in
- This helps us get candidates presented quicker as we all know these orders open/close quick
- This form needs to be 100% accurate and have all request noted in the highlights section
- Start Date should be on or very quickly after the Order Start Date listed
- All RTO should be listed on the cover page, must be less than 7 days
- RTO will not be approved in the first week of assignment or holidays
