Description Conduct thorough assessments of patients in various settings, devising personalized treatment plans that cater to individual needs Implement physical therapy techniques focused on enhancing mobility, strength, balance, and overall functionality Engage collaboratively with nurses, physicians, case managers, and fellow rehabilitation professionals to achieve optimal patient outcomes Guide patients and their families through therapeutic goals, discharge strategies, and home exercise regimes Document patient evaluations, treatments, and progress in adherence to facility protocols and regulatory standards Participate in interdisciplinary team rounds and conferences to ensure a seamless continuum of care Education
Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution Certifications
Current state licensure as a
Doctor of Physical Therapy CPR certification is preferred Additional specialized certifications (e.g., in geriatrics or neurology) are advantageous, yet not mandatory Skills
Prior experience in a clinical setting is highly desirable Proficient in treating patients with complex medical and mobility challenges Excellent communication and teamwork abilities Comfortable using digital charting and clinical record-keeping systems Benefits
