In this role, you will work with a diverse patient population, including those with general orthopedic conditions (hips, knees), generalized weakness, strengthening needs, and cardiac rehabilitation. You will have the opportunity to positively impact patients' lives through tailored rehabilitation programs and personalized care. Responsibilities: - Assess and evaluate patients to develop individualized treatment plans.- Implement effective physical therapy interventions to promote healing and functional independence.- Collaborate with a multidisciplinary team to coordinate patient care and progress.- Utilize the Epic documentation system for accurate patient record-keeping.- Maintain open communication with patients and their families to promote understanding and compliance.- Stay updated on best practices and advancements in physical therapy to ensure the delivery of high-quality care. Qualifications: - Valid Physical Therapist license for the state of West Virginia.- Basic Life Support (BLS) certification.- Strong clinical skills with experience in home health settings preferred.- Familiarity with Epic or willingness to learn the system.- Positive attitude and willingness to collaborate and learn. Scheduling & Environment: - Self-scheduling is permitted, allowing flexibility in managing your caseload.- No weekend or holiday requirements. Dress Code: - Navy scrubs or khaki bottoms with navy polo. Orientation: - Orientation will be provided at the Gassaway office with tailored training based on clinician needs, including Epic documentation.
