Harbor Care is seeking a talented and committed Director of Quality and Transformationto help further our mission. The Job: The Director of Quality and Transformation oversees quality improvement initiatives across Harbor Care. The Director of Quality and Transformation leads the Clinical Quality Management Program, and provides support to programs to advance quality improvement efforts. This is an autonomous position that will report to the Compliance Officer and collaborate with the Medical Director and Operations. The Director of Quality and Transformation strives to ensure compliance with all applicable federal, sponsor, and Harbor Care regulations and guidelines. Duties & Responsibilities Across all Harbor Care Programs and Services Develop and maintain dashboards, reports, and data visualizations to support decision making and performance tracking Collaborate with the Information Technology and Systems (ITS) team to e nsure data accuracy, integrity, and security in alignment with organizational policies and compliance standards Collaborate with Operations and clinical teams to use data to i dentify trends, patterns, and opportunities. Engage teams to enhance organizational efficiency and program effectiveness and develop plans of corrective action for measures falling below expectations. Oversee policies and procedures related to quality improvement Lead and advise program and administrative staff in performance improvement initiatives including PDSA cycles, safety and risk assessments, and other processes in support of safety, effectiveness, and efficiency Provide guidance and in-service training as necessary regarding quality improvement and data analytics Administer patient/client satisfaction surveys and communicate results to drive positive change Lead the process for recognitions and accreditations related to quality improvement Participate in committees, meetings, and trainings as they relate to quality improvement and data analytics Assist Compliance Officer with documentation, analysis, and resolution of patient/client complaints, near misses, and incidents At the Federally Qualified Health Center (FQHC) Facilitate FQHC QI Committee and Workplan and communicate activities and performance to staff and governing boards Utilize CMS Quality Metrics and other standardized tools for reporting clinical quality measures, including analyses of where data originates in the electronic medical record Oversee process for Peer Reviews Responsible for the preservation of all health center documentation related to any actual or potential claim or complaint (for example, medical records and associated laboratory and x-ray results, billing records, employment records of all involved clinical providers, clinic operating procedures); and Responsible for any service-of-process/summons that the health center or its provider(s) receives relating to any alleged claim or complaint is promptly sent to the HHS Office of the General Counsel, General Law Division, per the process prescribed by HHS and as further described in the FTCA Health Center Policy Manual. Job Requirements Basic Qualifications: Minimum 4-5 years progressive experience in quality improvement in a healthcare facility, preferably within a federally qualified health center Demonstrated skills in data collection, data analysis, and data visualizations Excellent oral and written communication skills needed, strong organizational ability required Experience in the use of spreadsheets, Power BI, or other advanced data visualization tools Knowledge of HRSA regulations and compliance requirements Preferred Qualifications: Bachelors of Nursing Degree or Master in Public Health preferred but individuals with experience in leading a quality improvement program may be considered Project Management skills Experience with underserved populations helpful and knowledge of health disparities highly desired Competencies Passionate about the mission and the communities served and sensitive to communities’ concerns Energetic, positive and committed to excellence Recognized for integrity; thoughtful, honest and fair Flexible and open to new ways of doing things.
