Full Time Speech Therapist

The Staff Speech Therapist evaluates and treats patients/residents, communicates with families, physicians, and other health team members, and maintains documentation of services in the medical records. Provides Speech Therapy services including treatment planning & implementation, and discharge planning. Tests patients' physical abilities and analyzes rehabilitation goals; consults with the rehab team to coordinate the most effective physical therapy program. Organizational Relationships:

Supervises: Responsible for the care delivered by SLPAs, Rehab Technicians. Evaluate patients/residents within 24 hrs of physician referral. Develop effective treatment plans and obtain approval from the referring physician; treat patients/residents according to the physician’s treatment plan. Communicate regularly with supervisor, other members of the rehabilitation team, and nursing staff on treatment plans, progress, and prognosis. Record evaluations, daily treatment notes, weekly progress notes, 14-day progress reports to the physician, and discharge summaries as per policy and procedure. Instruct patients'/residents' families or nursing staff in maintenance programs. Secure necessary durable medical equipment (e.g., orthoses, walkers, canes, elevated toilet seats, grab bars) to facilitate independence in mobility. Submit logs and billing information promptly. Report any equipment or problem areas within each facility to the supervisor immediately. Comply with the State Licensing Board of California, Title 22, CMS, and California Labor Board Guidelines. Licensure/Certification: Bachelor's degree (or foreign equivalent) in Speech Therapy and a current license or recent application for licensure.
