Job Description
The Staff Physical Therapist evaluates and treats patients/residents, communicates with families, physicians, and other health team members, and maintains documentation of services in the medical records. Provides physical therapy services including treatment planning, implementation, and discharge planning. Tests patients' physical abilities, analyzes rehabilitation goals, and consults with the rehab team to coordinate effective physical therapy programs. Organizational Relationships: Supervises: Responsible for the care delivered by PTAs and Rehab Technicians. Evaluate patients/residents within 24 hours of physician referral. Develop effective treatment plans and obtain approval from the referring physician; treat patients/residents according to the physician’s treatment plan. Communicate regularly with supervisors, rehabilitation team members, and nursing staff regarding treatment plans, progress, and prognosis. Record evaluations, daily treatment notes, weekly progress notes, 14-day progress reports, and discharge summaries per policy. Instruct patients'/residents' families or nursing staff in maintenance programs. Secure necessary durable medical equipment such as orthoses, walkers, canes, elevated toilet seats, and grab bars to facilitate independence. Submit logs and billing information promptly. Report any equipment or problem areas to the appropriate supervisor immediately. Comply with California State Licensing Board, Title 22, CMS, and California Labor Board guidelines. Licensure/Certification: Bachelor's degree (or foreign equivalent) in Physical Therapy, with a current license or recent completion of licensing exams.
