SCO Family of Services
· Quarterly Visits with the individual and the Family Care home.
· Acts as a liaison and advocates on behalf of the individual and the Family Care Provider to the health care professionals either via telephone and/or accompanying the individual and Family Care Provider on scheduled appointments
· Ensures that the individual is referred for routine, and/or specialized health services as needed, and that such services are provided in a timely manner,
· Completes Quarterly Nursing Assessments for each individual.
· Completes an Annual Plan of Care for each individual.
· Requests and reviews a copy of consultation reports, and all other pertinent medical documents, specialty medical services.
· Reviews health care documentation to ensure that all necessary examinations and paperwork for each health care provider is completed and up-to-date.
· Conducts training, as needed, for the individual and Family Care Providers to enable them to access, and/or provide appropriate care.
· Reports concerns of safety issues to the program administration, home liaison, service coordinator and any other staff as necessary.
· Ensures health records are being safeguarded, the individual’s rights to expect reasonable continuity of care, privacy concerning his/her own health care, and confidentiality of all communications.
The Family Care Nurse must possess good communication and interpersonal skills. The Nurse must be able to interact with a variety of health care professionals in various disciplines. The Family Care Nurse must be able to form relationships with individuals, families, Family Care Providers, staff, health care professionals, and any other outside service provider as necessary.
