In addition to behavioral health care, Metrocare provides primary care centers for adults and children, services for veterans and their families, accessible pharmacies, housing, and supportive social services. The Licensed Professional Counselor is primarily responsible for monitoring services and providing Community Support Services and therapeutic services. The LPC uses a basic knowledge of diagnostic characteristics, symptomology of primary mental illness and knowledge of medications, side effects, and benefits to provide services in a person-centered, supportive, community-based environment. ESSENTIAL DUTIES AND RESPONSIBILITIES: Provides individual and small group training in living skills such as social skills, budgeting, mobility training, grocery shopping, menu planning, cooking, coping skills, personal hygiene, personal organization, home maintenance, etc. Provides therapeutic counseling services in accordance with clinical algorithms. Assists in completing intake assessments and diagnostic formulations as needed. Coordinates services within the Agency and with other community providers. Assists with the involvement and education of the consumer's family at a level acceptable to the consumer. Assists with monitoring and continued assessment of assigned consumers, which may require home visits. Responds to crisis or potential crisis situations and ensures the appropriate level of care through the coordination of services or the provision of community support services. Facilitates skills training and therapeutic groups. Documents services in the automated clinical record keeping system within 24 hours after services have occurred. Maintains required productivity levels as outlined in Center policies. Maintains confidentiality of information concerning consumers and family members. Completes a psychosocial assessment on each new admission, with 100% compliance expected. Formulates individual service plans based on assessment findings on all new admissions, with 100% compliance expected. Develops measurable objectives and goals agreed upon by the consumer. Participates in interdisciplinary team staffing and updates the team on progress. Provides crisis intervention and facilitates inpatient admission upon request. Performs other duties as assigned. QUALIFICATIONS EDUCATION AND EXPERIENCE: Masters degree in Counseling, Psychology, or related field with 2 to 3 years of specialized experience. FULLY LICENSED PROFESSIONAL COUNSELOR COMPETENCIES: Conducts job responsibilities in accordance with ethical standards and applicable laws. Excellent communication and organizational skills. Able to work autonomously with minimal supervision.
