In addition to behavioral health services, Metrocare provides primary care centers for adults and children, accessible pharmacies, housing and supportive social services. Through the Altshuler Center for Education & Research we are advancing mental health beyond Dallas County.****JOB DESCRIPTION****GENERAL DESCRIPTION:**

The mission of Metrocare Services is to serve our neighbors with developmental or mental health challenges by helping them find lives that are meaningful and satisfying. We are an agency committed to quality gender-responsive, trauma-informed care to individuals experiencing serious mental illness, development disabilities, and co-occurring disorders. Metrocare programs focus on the issues that matter most in the lives of the children, families, and adults we serve.The LPHA is primarily responsible for monitoring services and providing Community Support Services and therapeutic services. The LPHA uses a basic knowledge of diagnostic characteristics, symptomology of primary mental illness and knowledge of medications, side effects and benefits to provide services in a person-centered, supportive, community-based environment. The LPHA is monitored by periodic supervision.**ESSENTIAL DUTIES AND RESPONSIBILITIES:**

The essential functions listed here are representative of those that must be met to successfully perform the job.* Provides individual and small group training in living skills such as social skills, budgeting, mobility training, grocery shopping, menu planning, cooking, coping skills, personal hygiene, personal organization, home maintenance, etc.* Provides therapeutic counseling services in accordance with clinical algorithms.* Assists in completing intake assessments and diagnostic formulations as needed.* Assists with the involvement and education of the consumer’s family at a level acceptable to the individual.* Responds to crisis or potential crisis situations and ensures the appropriate level of care.* Documents services in the automated clinical record keeping system with 24 hours after services have occurred.* Maintains required productivity levels as outlined in Center policies.* Maintains confidentiality of information concerning consumers and family members.* Provide overall service coordination, referral, linkage, and advocacy to consumers with varying needs. Perform duties in concert with other members of an interdisciplinary treatment team.* Formulate individual service plan based on assessment findings on all new admissions. 100% compliance expected.* Develop measurable objectives and goals agreed upon by the individual.* Review service plans every three months or as clinically indicated. 100% compliance expected.* Participate in interdisciplinary team staffing**COMPETENCIES/SKILLS:*** Analytical skills, professional acumen, business ethics, thorough understanding of continuous improvement processes, problem solving, respect for confidentiality, excellent communication and presentation skills.* Conducts job responsibilities in accordance with the ethical standards of conduct, state contract, appropriate professional standards, and applicable state/federal laws**QUALIFICATIONS****EDUCATION AND EXPERIENCE:**

Master’s degree in Counseling, Psychology, or related field with 2 to 3 years of specialized experience, LPC, LCSW required.**REASONING ABILITY:*** Ability to operate as a team member, yet able to make positive, individual judgments.* Ability to assess a potential crisis situation and ensure the delivery of services at the proper level of care.* Ability to work in a high stress environment, take initiative and be creative.**CERTIFICATIONS, LICENSES, TRAINING, REGISTRATIONS:*** Full licensure as LPHA, LPC, LCSW.* TB Test, CPR, First Aid and SAMA**PHYSICAL DEMANDS & WORK ENVIRONMENT****PHYSICAL DEMANDS:**

Due to the multi-site responsibilities of this position the incumbent must be able to carry equipment and supplies (up to 15 pounds). Positions that are “community-based,” in whole or part, require the incumbent to travel between various worksites within his/her workday/workweek. The incumbent is required to have reliable transportation that can facilitate this requirement. The incumbent is further required to meet the criteria for insurability by the Center’s risk management facilitator; and produce proof of minimal auto liability coverage when applicable. Failure to meet these terms may result in disciplinary action up to and including termination of employment, contract or other status with Metrocare.**WORK ENVIRONMENT:**

Option 1: Monday 5pm to 10pm; Wednesday 5pm to 10pm; alternating Friday 5pm to 10pm; Saturday 8am to 2pm
