Heritage Health Network (HHN) is a fast-growing provider of Enhanced Care Management (ECM) and Behavioral Health services under California’s CalAIM initiative. We partner with Medi-Cal Managed Care Plans, hospitals, IPAs, community-based organizations, and faith-based groups to deliver whole-person care for underserved populations. Community Partnerships Manager (Riverside & San Bernardino Counties) Position Overview

The Community Partnerships Manager will build HHN’s Community Care Hub, a network of CBOs, FBOs, shelters, and clinics aligned under shared goals, technology, and reporting. This role is responsible for developing referral partnerships, executing MOUs, and creating strong cross-sector collaborations to expand HHN’s reach and strengthen community-based care coordination. This position combines relationship management, outreach, and structured partnership building. Success is measured by executed partnership agreements and referral growth into HHN’s ECM program. Key Responsibilities

Partnership Development: Identify, engage, and contract with community-based organizations (CBOs), faith-based organizations (FBOs), shelters, and clinics.

Negotiate and execute MOUs to formalize referral and collaboration agreements.

Build trust with community leaders to integrate services into HHN’s Community Care Hub model.

Program Integration: Connect partner organizations into HHN’s shared technology platform for referrals and reporting.

Support partners in aligning with Medi-Cal ECM workflows and documentation standards.

Ensure that community partners understand and use HHN’s reporting systems to track impact.

Community Outreach & Representation: Represent HHN at community coalitions, health fairs, and local convenings.

Build visibility with grassroots organizations and advocacy groups.

Serve as a liaison between HHN and community agencies to ensure alignment of services.

Performance & Reporting: Maintain pipeline of potential and signed partnerships in CRM.

Deliver weekly progress reports on new MOUs, referral activity, and partnership outcomes.

Provide feedback to leadership on community needs, barriers, and opportunities.

Qualifications

Bachelor’s degree in Public Health, Social Work, Community Development, or equivalent experience.

3+ years of experience in community partnerships, nonprofit management, or healthcare outreach.

Strong knowledge of community-based services, FBOs, and safety net providers.

Excellent communication, relationship management, and negotiation skills.

Experience with Medi-Cal, CalAIM, or ECM preferred but not required.

Ability to travel regularly throughout Southern California.
