Day (United States of America) Job Summary
The Care Partner is a support care provider who functions under the direction of the Registered Nurse. The primary focus is to promote a positive patient experience by partnering with the nurse, physician, healthcare team and other providers to deliver excellence in patient experience; and to assist with daily living activities and comfort measures. The Care Partner performs basic patient care functions (e.g., ADLs, ambulation, and vital signs) and may require approved advanced technical skills as needed (e.g., phlebotomy, dressing changes, oral suctioning, discontinuing Foley catheters). Clerical duties such as data retrieval, maintaining unit supplies, and transporting supplies/patient medications may be required. The role requires the ability to work in a fast-paced environment, multi-task, and communicate effectively with the RN and the care team, while maintaining professionalism and compassion. The exact advanced skills required will depend on unit needs and may fluctuate. The Care Partner must be able to take direction from various care providers and maintain strong organizational and communication skills. Core Responsibilities And Essential Functions
Provide excellence in patient experience by partnering with the healthcare team, patient and family. Support patient-centered clinical care, address patient needs, follow the plan of care, and engage patient and family in care decisions as appropriate. Contribute to the plan of care with focus on safety and the care continuum; implement safety interventions to minimize risk. Protect patient rights and dignity; maintain confidentiality per HIPAA. Participate in hourly patient rounding; respond to telephone calls and call lights promptly and professionally. Escalate concerns to the charge nurse when needed; offer comfort measures to patients and families. Present professionally and respectfully, with sensitivity to others’ concerns and needs. Provide basic patient care under RN direction (hygiene, nutrition, elimination, comfort) and perform unit-specific advanced skills as required (e.g., phlebotomy, EKG, catheter removal). Assist in patient admission and data entry; monitor key patient safety information. Participate in charge reconciliation and unit audits; support unit performance improvement initiatives. Transport supplies, medications and equipment; maintain equipment and report issues. Use effective communication with patients, staff, physicians, families and visitors; document care in EMR; communicate any concerns in real time. Maintain infection control and safety practices; follow PPE and universal precautions; ensure clean and safe patient environments. Required Minimum Education
Rising Senior in Good Standing High School Diploma (General) or GED (Preferred) or Medical Assisting Certificate (Preferred) Required Minimum License(s) And Certification(s)
BLS - Basic Life Support (Instructor or standard) CNA - Certified Nursing Assistant (Preferred) Additional License(s) And Certification(s)
All certifications are required upon hire unless otherwise stated. Required Minimum Experience
Must be 16 years of age or older. Required Minimum Skills
Ability to follow detailed directions Strong interpersonal skills Clear verbal and written communication, including documentation Ability to work independently and as part of a team Wellstar minimum age of hire is 16 Seniority level
