The Care Partner is a support care provider who functions under the direction of the Registered Nurse. The Care Partner primary focus will be to promote a positive patient experience by partnering with the nurse, physician, healthcare team and other healthcare providers to provide excellence in patient experience; and assist with daily living activities and comfort measures. The Care Partner assists other health care providers with safe, age appropriate and culturally aware care by performing basic patient care functions; i.e. ADL\'s, ambulation, and vital signs. The role may require advanced technical training skills (e.g., phlebotomy, simple treatments, dressing changes, oral suctioning, discontinuing Foley catheters). Performing additional advanced skills may be required to support clinical care and will fluctuate depending on unit needs. Clerical tasks such as data retrieval, maintaining unit supplies, and transporting supplies/patient medications may be required. The Care Partner must be able to work in a fast-paced environment, multi-task, and communicate effectively with the Registered Nurse and ancillary team. Must be able to perform under stressful conditions with a compassionate, respectful and service mindset. Must be able to take direction from a variety of care providers and be organized with excellent communication skills. Core Responsibilities And Essential Functions

Excellence in Patient Experience by partnering with health care team, patient and family. Patient Centered Care-Clinical Practice: support an environment of healing or wellness, address patient needs, follow the plan of care as assigned, engage patient and family in compliance with the treatment plan, contribute to the plan of care in collaboration with the healthcare team, and implement safety interventions to minimize risk. Protects rights and dignity of patient. Responsibilities

Maintains confidentiality adhering to HIPAA regulations and privacy procedures. Participates in hourly patient rounding. Answers telephone and call lights promptly and professionally; notifies appropriate individual of follow-up needs. Resolves patient concerns or escalates to the charge nurse as needed. Proactively offers assistance and comfort measures to patients and families. Present self in a positive and professional manner and show sensitivity to concerns and needs of others. Patient Centered Care

Provides patient care under the direction of the RN. Addresses patient care needs (hygiene, nutrition, elimination, physical comfort) and additional advanced skills based on unit requirements (e.g., phlebotomy, EKG, removal of Foley catheters, removal of IVs). Answers telephone and call lights promptly, professionally and notifies appropriate individual of follow-up. Assists RN and other team members with care delivery as assigned. Assists in direct patient admission. Monitors data entry for key patient safety information. Participates in charge reconciliation and performs audits as requested. Teamwork

Supports initiatives of the healthcare team and unit performance improvement. Supports professional nursing practice through shared leadership, evidence-based practice and ethical accountability. Transports unit supplies, patient care supplies, medications and equipment as assigned. Communication

Uses effective written and oral communication with patients, staff, physicians, family and visitors (AIDET). Uses safety communication techniques and follows chain of command when necessary. Communicates completion of assignments to RN or Charge Nurse and documents care in the EMR. Notifies RN/Charge Nurse of patient care concerns in real time; maintains positive, respectful communication with the care team, patients and families. Maintains and reports on patient care documentation tools during downtime. Safety and Infection Control

Provides a safe environment for patient care and maintains proper hand hygiene. Keeps patient rooms and unit areas clean and free of clutter. Follows PPE and infection control policies and reinforces universal precautions with patients and families. Uses proper body mechanics and assistive devices; performs environment of care rounds. Performs other duties as assigned and complies with Wellstar Health System policies, standards of work, and code of conduct. Qualifications

Rising Senior in Good Standing or High School Diploma or GED – Required Basic Life Support from American Heart Association – Required Certified Nurse Assistant – Preferred Required Licenses and Certifications
