JOB TITLE: Social Worker II
REPORTS TO: Supportive Services Manager

STATUS: Regular, Full-Time, Non-exempt
WAGE RANGE: $32.00 - $36.06 /hour
LOCATION: 50% at 555 Larkin St, San Francisco, CA 94102; 50% at 864 Ellis Street Apartments San Francisco, CA 94102
SCHEDULE: Monday - Friday; 8:30 AM - 5:00 PM; 4 Days onsite, 1 day remote
Summary
Under the general direction of the Social Worker/Site Supervisor the Social Worker is responsible for assisting residents living in TNDC properties with housing stabilization & retention by providing counseling and support services, community outreach, advocacy, information, and referrals to existing off-site providers. Depending on the building or portfolio language proficiency in Cantonese, Mandarin, Russian or Spanish verbal, reading, and writing skills may be required. TNDC supports the professional growth of staff in the Tenant and Community Services Department- Social Work Unit, including pursuing licenses (LCSW, LPCC, MFT). Details provided upon hire. Responsibilities
Deliver culturally inclusive support service programs for tenants. Manage a caseload of individuals. Outreach to assigned tenants, including home visits. Identify tenants in need of services and monitor health status, including medical, mental health and substance use needs. Complete comprehensive Intakes and Needs Assessments. Develop Individualized/Family Oriented Service Plans with tenant/family driven goals. Provide trauma-informed case-management and supportive counseling. Provide housing stabilization, retention and eviction prevention services. Provide Crisis Intervention and conflict resolution. Provide information and referrals to off-site service providers and community programs. Provide referrals to off-site Out of School Time programs. Support transitions to community-based resources and assist with exit planning and up to 30 days of after-care when appropriate. Organize and facilitate community-building activities and workshops. Collaborate with Property Management on housing retention and eviction prevention. Provide follow-up information to Property Management staff as needed. Act as liaison/advocate for tenants with Property Management and off-site service providers. Attend weekly Property Management meetings and on-site Tenant Meetings; participate in monthly off-site community meetings and other scheduled meetings. Develop new resources and relationships with outside service providers. Comply with mandated reporting (Adult Protective Services & Child Protective Services). Assist families with reunification and family sustainability. Comply with HIPAA and PHI policies and maintain confidentiality. Maintain accurate and up-to-date documentation of tenant services using TNDC’s CIRCE database and SF City & County Clarity ONE System. Assist with processing intakes for targeted LOSP and S+C referrals through San Francisco’s Coordinated Entry System, in partnership with TNDC’s Property Management. Carry and respond promptly to cell phone during business hours. Other duties as assigned. Qualifications
REQUIRED SKILLS
Knowledge of and sensitivity to issues of homelessness. Knowledge of Trauma Informed Care approaches. Excellent knowledge of substance abuse and mental health issues. Excellent crisis intervention skills. Willingness and ability to practice cultural humility. Ability to work with culturally diverse, low-income populations. Strong analytical skills. Ability to handle multiple tasks simultaneously. Initiative and creativity. Computer proficiency: Microsoft Office. Ability to maintain confidentiality. Strong leadership and communication skills. Ability to work as part of a multi-disciplinary team. Excellent benefit advocacy skills and problem-solving skills. MINIMUM QUALIFICATIONS
Bachelor’s Degree in Social Work or related field. Two years of experience with very low-income populations in social services. Two years of direct service experience with individuals with substance use and mental health issues and/or co-occurring disorders. Two years of experience linking individuals to medical, mental health and other services. One year of experience working with seniors, families, or individuals depending on building/portfolio. Knowledge of San Francisco and Greater Bay Area resources and benefits programs. PREFERRED QUALIFICATIONS
Experience in Harm Reduction. Experience in Permanent Supportive Housing. Experience providing housing stabilization, retention and eviction prevention.
