Behavioral Health Care Coordinator - Remote

Role Overview: Provides care coordination for members with behavioral health conditions requiring intensive intervention and oversight, utilizing clinical, social, and community resources. Promotes the appropriate use of clinical and financial resources to improve care quality and member satisfaction. Essential Functions: Conduct comprehensive health risk and needs assessments covering psycho-social, physical, medical, behavioral, environmental, and financial factors. Develop and communicate treatment plans for service authorization and serve as the point of contact to ensure appropriate service delivery, including during transitions to home care and community-based services. Qualifications: Must have 5 years of post-Master’s experience. Required licenses: LCSW-C, LCPC, or LCMFT. Work hours: Monday-Thursday 8 am - 7 pm, Friday 8 am - 6 pm, with flexibility for two evening shifts per week (until 7 pm Mon-Thu or until 6 pm on Friday). Training period: 3 weeks, Monday-Friday, 8:30 am - 5:00 pm. Post-training, schedules will be fixed between the 4th and 6th week. All information will be kept confidential according to EEO guidelines.
