The County of Del Norte Department of Health and Human Services - Behavioral Health Branch is seeking a qualified candidate for the position of Client Services Specialist I/II. This position is classified as full-time.

Under direct supervision performs a variety of routine to complex work in Department of Health & Human Services (DHHS) programs and related administrative functions. Duties listed below may vary depending on program placement. Provide outreach, and referral services within the community; may perform case management; tracks and monitors client needs through interactive interviews and fact gathering to ensure compliance with the program guidelines and regulations.

Incumbents in this class serve as a liaison between clients and community agencies and resources. This position functions as an integral and supportive member of the assigned departmental team.

Supervision Received and Exercised: Employees within this classification receive general supervision within a framework of policies and procedures. Receives direct supervision from the Appointing Authority or designee as assigned; May provide supervision over program staff as assigned. Detailed Work Activities

Essential job duties may include any of the following tasks, knowledge, skills and other characteristics. Incumbent(s) may not be required to perform all duties listed, and may be required to perform additional, position-specific tasks. Establish trusting relationships with clients and their families while providing general support and encouragement. Participate in required training programs. Provide consistent communication about the individual client's progress to evaluate client status. Work collaboratively, participate in case conferencing to provide consultation and make recommendations Compose and prepare correspondence in accordance with department policy. Enter and retrieve information into a computer terminal; use the computer system to retrieve a variety of daily, monthly and yearly reports. Provide transportation for appointments, interviews, court hearings, etc. Completes special projects as directed by the supervisor or program manager including data collection and data entry. Facilitate communication and coordinate services between providers and the client. Work collaboratively with other clinical and support staff to meet the needs of clients and families. Document client care management information in the Electronic Health Record and other software within agency timeliness standards. Visit public parks, homeless camps, community locations, and client homes in order to contact individuals to help engage them into services, while developing relationships with shelters and other community programs that provide direct support services to homeless persons. Advocate on behalf of clients; act as a liaison to link clients with community services/agencies, peer support groups, legal system, financial agencies, schools, families, housing, etc. by interviewing and assessing client's needs, making referrals, providing supportive assistance and assisting the client in negotiating and securing placement with services. Monitor service delivery and help identify gaps in service delivery. Write chart notes in accordance with professional standards, state regulations and department protocol, and ensure proper filing either by filing or submitting to the proper area for filing. Provides back up clerical support in ensuring client files are maintained; that department mail, faxes, and client records are distributed and filed according to department policy. In addition, perform other reception activities; answer phones; respond to inquiries; provide information to the public and other agencies. Assist clients in daily living activities which may include personal care services. Assist in the safeguarding of client assets/belongings. Provide ongoing follow-up, basic motivational interviewing, and goal setting with clients and families. Assist clients in utilizing health and social resources, including scheduling appointments, assisting with completion of applications for eligible programs, and transportation to appointments. Performs related duties as required or assigned Minimum Qualifications

Any combination of education, training and experience which would likely provide the required knowledge, skills and abilities may be considered qualifying. A typical way to obtain the required knowledge and abilities would be:

Client Services Specialist I: Pattern 1: High school diploma or GED equivalent and one (1) year of varied community outreach or customer service experience which involves frequent public contact. OR Pattern 2: High school diploma or GED equivalent and six (6) months of experience working in a social service, mental health, substance use disorder or public health setting. If assigned to Public Health: Written and oral fluency in English and Spanish or Hmong is preferred.

Client Services Specialist II: Pattern 1: A minimum of one (1) year of experience as a Client Services Specialist I. OR Pattern 2: Two (2) years of experience working in a social service, mental health, substance use disorder or public health setting.

Desirable Qualifications: Behavioral Health: Associates Degree in Social Sciences or related field or valid registration for addiction counseling with a qualifying certified body.

If assigned to Public Health: Written and oral fluency in English and Spanish or Hmong is preferred.

Special Requirements: Must possess and maintain a valid Driver's license.

Environmental, social, economic, cultural, and other issues and recent developments impacting the health of individuals and the community. Ability to: Understand and accept differences in human behavior and promote cultural competency in serving vulnerable populations. Read, interpret, and follow departmental policies and procedures. Operate standard office equipment. Establish and maintain effective working relationships with fellow workers, outside agencies and/or the general public. Organize workload, set priorities, meet deadlines and follow up assignments with minimum of direction. Work a flexible schedule, which may include working outside of business hours. Demonstrate sensitivity to and understanding of historically minoritized populations and participate in professional development activities to increase cultural competency and health equity practices. Provide information to the public on services offered. Coordinate referrals to other agencies. Write clear and concise chart notes in accordance with professional standards, state regulations and department protocol. Analyze crisis situations and report promptly. Communicate medical information to healthcare professionals and care coordinators. Advocate for vulnerable populations within the healthcare system and in the community. Assist in the planning and delivery of client care.

Selection Process:

ELIGIBILITY:

Regular Permanent Full-Time Employees : Eligible for a comprehensive range of benefits

Regular Permanent Part-Time Employees : Eligible for select benefits on a prorated basis, depending on hours worked

Temporary Employees : Not eligible for company benefits Paid annual leave: Vacation: 1-5 years of continuous service: 15 working days per year; 6-10 years of continuous service: 20 working days per year; 11-15 years of continuous service: 25 working days per year; 16 years or higher of continuous service: 30 working days per year.
