Nurse Practitioner/Physician's Assistant
In search of a Licensed Nurse Practitioner/Physician’s Assistant to work in our outpatient Medication Assisted Treatment Program. This position will task the provider with seeing patients in a medically-assisted recovery program. The Nurse Practitioner/Physician’s Assistant provides Substance Abuse care to the Opioid Use population, including formulating a diagnosis and a treatment plan and providing continued follow-up patient care; prescribing and monitoring medication used in the Opioid Treatment Program.
ESSENTIAL DUTIES
- Provide direct primary and acute addiction care services to adult patients
- Perform initial and on-going assessment of patients' medical, physical and psychosocial status
- Obtain and evaluate health and medical history; perform a physical exam, conduct preventive screening procedures based on age and history
- Identify medical and health risks and needs
- Complete accurate and timely documentation of observations, interventions, plan of care, referrals and follow-up
- Identify needs of the individual and develop a plan of care and/or education plan
- Make appropriate referrals to other health professionals and community agencies
- Reassess and modify the plan as necessary to achieve medical and health goals
- Monitor co-morbid issues and coordinate with primary care and other specialty agency to ensure best practices
- Provide medical guidance to the program regarding policy and procedures.
- Participate and engage in staffing and care planning meetings regarding patient progress and care.
QUALIFICATIONS:
- Licensed Nurse Practitioner or Physician Assistant able to practice in the state of VA
- Advanced nursing knowledge and clinical skills through an appropriate educational program
- Minimum of 1 year of experience as a practicing nurse practitioner
- DEA License
- Willingness and ability to get X-Waiver
- Ability to work early hours to meet the needs of our clients
- Experience in addiction/psychiatric treatment preferred
