Position Overview
Navigating healthcare today is overwhelming, especially for patients living with chronic conditions. At Aviator, we’re building a new model of support that combines human expertise with technology to make care more accessible and coordinated. Nurse Practitioners are central to this effort.
In this role, you’ll work closely with our patient navigators and physicians to deliver seamless, high-quality care. You’ll provide direct clinical support, develop and guide care plans, and help patients manage the challenges of complex health needs. As part of Aviator’s clinical team, you’ll also play an active role in refining and scaling our care model so it delivers both strong clinical outcomes and a better experience for families.
Responsibilities
- Deliver direct patient care through virtual visits.
- Partner with healthcare navigators and physicians to build and oversee holistic care plans.
- Assess, diagnose, and manage common chronic conditions within scope of practice.
- Identify and prioritize patient needs, ensuring continuity of care.
- Integrate social determinants of health (e.g., food, transportation, housing) into patient plans.
- Contribute to building Aviator’s new patient care program, helping us refine and expand our model.
- Use technology and AI-enabled systems to deliver care efficiently and effectively.
Qualifications
- Current Nurse Practitioner licensure and national certification (FNP, AGNP, or similar).
- Current, active, and unencumbered Nurse Practitioner license with Full Practice Authority in one of the following states: Maryland, Washington, Arizona, Colorado, Minnesota, Oregon, Iowa, Kansas, Nevada, Nebraska, New Mexico, Utah, New Hampshire, West Virginia, Idaho, Montana, Delaware, Hawaii, Maine, South Dakota, Vermont, Alaska, Wyoming, North Dakota, Rhode Island.
- Experience providing care for patients with chronic conditions.
- Strong clinical judgment and ability to coordinate across multiple providers.
- Excellent communication and empathy skills; you listen first and guide second.
- Comfortable with technology and able to quickly learn new systems.
- A proactive, solutions-oriented mindset; you thrive in fast-paced, evolving environments.
