Social Worker -- Southmont
Washington, PA – Part-time, varied schedule
Southmont is the skilled nursing community on our Washington, PA campus. At Southmont, our modern, home-like atmosphere provides a comfortable and secure healing environment. Arranged as neighborhoods, our skilled nursing areas offer a wide range of care programs and services for those in need of long-term care. Expert registered nurses, licensed practical nurses, certified nursing assistants, and social workers partner with residents, their families, and their doctors to provide personalized care plans. Southmont also provides short-term rehabilitation that can include physical, occupational, and/or speech therapies.
The Social Worker responds to individual resident social needs from the time of admission and throughout the resident’s stay. The role collaborates with other staff and professionals both in and outside of the facility to develop and implement coordinated, individualized plans of care and other supportive services.

Requirements for the Social Worker:
• Holds a Bachelor’s degree from an accredited college or university, or four years of relevant experience in a long-term care community, or has an equivalent combination of education and experience. A degree in Social Work, Gerontology, or another related field is preferred.
- Social Work licensure in PA is preferred.
- Knowledge of Microsoft Office programs.
- Ability to work well with people and show enthusiasm and motivation.
- Self-directed and able to set priorities; excellent time management skills.
- Desire to contribute to our compassionate, resident-focused culture and effectively interact with residents, resident families, and team members.
