Base pay range

$70,000.00/yr - $75,000.00/yr

• Facility: Arnold Walter Nursing and Rehabilitation Center
• Position: Social Worker - Skilled Nursing Facility Experience Required
• Location: Hazlet, NJ
• Shift: Full Time
• Salary: $70,000 - $75,000 per year

Responsibilities

• Develop, implement, and evaluate social service programs for residents.
• Stay current with federal and state regulations, recommending policy updates as needed.
• Lead discharge planning, resident assessments, and individualized care plan development.
• Conduct interviews with residents and families to gather social histories and provide support.
• Coordinate transportation and connect residents with community resources.
• Educate residents and families on Medicare, Medicaid, and financial assistance programs.
• Collaborate with medical staff and interdisciplinary teams to meet residents’ needs.
• Maintain thorough, accurate, and timely progress notes.
• Support quality assurance efforts and assist during surveys and inspections.
• Actively participate in committee meetings (Infection Control, Policy Advisory, Quality Assessment).
• Ensure residents’ end-of-life care decisions (MOLST/POLST) are honored and documented.

Qualifications

• Experience: Minimum 1 year in social services, with nursing home experience required.
• Skills: Strong communication, interpersonal, and problem-solving skills.
• Ability to work collaboratively within a team environment.
