POSITION SUMMARY

The Social Worker will be an integral part of the Hospice Interdisciplinary Team, assisting in understanding the social, emotional, and cultural factors related to each patient's terminal illness. The Social Worker conducts psychosocial assessments and evaluations, providing emotional counseling to patients and families in their residence—both in person and telephonically. The role operates under the supervision of a physician and the Director of Patient Care Services, serving as a resource to patients, families, and the community. Must be comfortable using computers and electronic medical records.

QUALIFICATIONS

• Master of Social Work degree from an accredited school by the Council on Social Work Education.
• Clinical experience relevant to the psychosocial needs of patients and families.
• Excellent verbal and written communication skills.
• Spanish speaking skills are a plus but not required.
• Valid driver’s license, auto insurance, and reliable, well-maintained transportation.
