Full Time Medical SOCIAL WORKER, MSW

Hospice/Palliative Care
Coverage area includes the San Fernando Valley
Hourly Rate $35 + mileage reimbursement

Responsibilities

• The Social Worker will be an integral part of the Hospice Interdisciplinary Team and will assist the team in understanding the significant social, emotional and cultural factors related to each patient’s terminal illness.
• Conduct psychosocial assessments and evaluations, and provide emotional counseling to patients and families in their place of residence (in person and telephonically).
• Work under the direction of a physician and supervision of the Director of Patient Care Services, and serve as a resource to patients, families and the community.
• Utilize computers and be comfortable with electronic medical records.

Qualifications

• Master of Social Work degree from a school accredited or approved by the Council on Social Work Education.
• Clinical experience relevant to the psychosocial needs of patients and families.
• Excellent verbal and written communication skills.
• Spanish speaking a plus, but not required.
• Valid driver license and auto insurance and reliable/well maintained transportation.
