About the Role

This is a full-time, weekday Social Work position supporting medically complex, aging adults through the Program of All-Inclusive Care for the Elderly (PACE), a comprehensive senior care model designed to keep older adults living safely in their homes and communities. In this dynamic float role, you’ll provide clinical social work services, care coordination, and psychosocial support across multiple PACE clinic locations throughout Seattle and nearby areas, including home visits and community-based settings.

Responsibilities

• Conducting psychosocial assessments
• Providing one-on-one and group counseling
• Coordinating community and financial resources
• Advocating for participant needs across care settings, including home visits

Qualifications

• Master’s Degree in Social Work
• Active Washington licensure as one of the following:
• Clinical Independent Social Worker (or Temporary License)
• Clinical Independent Social Worker Associate (or Temporary License)
• At least 2 years of experience working with geriatric populations from diverse backgrounds

Required Skills

• Experience in social work with geriatric age groups from diverse ethnic cultures

Preferred Skills

• Mission-driven approach to social work
• Valuing long-term relationships and person-centered care

Pay range and compensation package

Salary Range: $36.81 – $57.15/hour

$5,000 hiring bonus for eligible external candidates
