Role Description

This is a full-time, on-site role for a Social Worker, based in Marble Falls, TX. The Social Worker will be responsible for providing emotional support and counseling to patients and their families, conducting assessments, developing care plans, and coordinating with healthcare professionals to ensure comprehensive care. They will also assist with resource linkage, facilitate support groups, and keep detailed records of patient interactions and progress.

Qualifications

• Experience in emotional support and counseling, particularly in hospice or palliative care settings
• Skills in assessment, care planning, and coordinating with healthcare teams
• Knowledge of community resources and experience in resource linkage
• Ability to facilitate support groups and maintain detailed records
• Strong communication and interpersonal skills
• Master's degree in Social Work (MSW) or related field
• Valid Social Work licensure in Texas
• Experience with electronic health records (EHR) systems is a plus
