Overview
The Aging & Disability Services | MnCHOICES Division within the Social Services Department is responsible for administering Home and Community Based Services. The division is comprised of three major functional areas: the MnCHOICES unit, Administration & Access unit, and the Case Management unit. It is a priority and expectation of the county and the department to operationalize racial equity across the administration of Home and Community Based Services, as well as grow an organizational culture that is value based: People, Integrity, Community, Equity, and Leadership; and embodies the Mission & Vision of Ramsey County.
The Social Worker position supports two areas: MnCHOICES (screening and face-to-face assessments using the MnCHOICES web-based tool to determine eligibility for EW, AC, CADI, CAC, BI, DD, Rule 185 services, and CFSS; develop a Community Support Plan; document in SSIS; identify supports; complete forms; make strength-based service recommendations; and participate in a multidisciplinary team) and Case Management (providing case management services to people with disabilities, developing person-centered Support Plans, coordinating services, monitoring outcomes, leading annual team meetings, and maintaining case documentation; working with multiple funding streams and specializing by area as appropriate).
Responsibilities
- For MnCHOICES: conduct screening and face-to-face assessments to determine eligibility for EW, AC, CADI, CAC, BI, DD, Rule 185, and CFSS; develop Community Support Plans; document assessment and referral activities in SSIS; identify potential support services and community resources; complete county and state forms; make recommendations based on assessment results using a strength-based approach; work in a multidisciplinary team.
- For Case Management: provide case management services to people with disabilities, identify needs for planning, seek out, acquire, authorize, and coordinate services; monitor and evaluate service delivery; advocate for rights of persons with disabilities; develop person-centered Support Plans; coordinate and monitor services; lead annual team meetings; maintain documentation in SSIS; work with multiple funding streams; specialize by area when applicable.
- Note that Case Managers typically specialize in a specific area (e.g., IDD CM, CCB CM, AC/EW CM).
- Complete tasks related to eligibility, treatment cost reimbursements, service referrals, and evaluation of client outcomes; maintain accurate case records.
Work Arrangement
Flexible Workplace: This position is identified as ‘flex work eligible’, allowing in-office or remote work with at least two days per week in-office and additional on-site expectations. The role carries schedule flexibility beyond the minimum policy and requires adherence to the county’s flexible workplace policy.
