Overview

We are currently searching for a Social Services Designee to join our friendly, caring and supportive team at Warren Barr Gold Coast in Chicago, IL. The facility is a premier skilled nursing environment focused on compassionate rehabilitative care and long-term services in a vibrant setting.

Responsibilities

• Assist in preparing residents for their future and ensure social and emotional needs are met during their stay.
• Coordinate resident discharges and participate in resident care conferences.
• Apply knowledge, skills, and attention to detail to ensure residents are comfortable and safe.

Qualifications

• As a minimum, a high school diploma or equivalent and two (2) years of experience in a Social Worker role, preferably in a healthcare setting.
• Prior Social Services Designee experience in a Skilled Nursing Facility, bachelor’s degree in Social Work or a related field, and five (5) years of related experience are preferable, but we are willing to invest in the right candidate.
