Social Worker role at Veterans in Healthcare (Yuma, AZ) within Onvida Health, located at Yuma Regional Medical Center. This is a Regular Full Time position with 80.00 hours every two weeks, daytime shift, hourly pay. Salary range: Min = $27.89, Mid = $34.86, Max = $41.83. The role supports the physician and interdisciplinary team in patient care, aiming to enhance clinical outcomes and patient satisfaction while managing cost of care and providing timely information to payers.

Responsibilities

• Conduct screening and assessments of patient’s psychosocial risk factors through evaluation of prior functioning levels and adequacy of support systems.
• Identify and manage victims of abuse or crisis situations (including child abuse/neglect, domestic violence, elderly abuse, institutional abuse, sexual assault) and follow hospital policy related to these issues.
• Communicate with patients and families regarding emotional, social, and financial impact of illness/disability; mobilize family/community resources; conduct family meetings to set social service goals and implement treatment plans.
• Educate patient/family and physician regarding post-acute options, adaptation to diagnosis, illness, treatment, and life situation.
• Participate in discharge planning for complex patients to ensure timely discharge and appropriate linkage with post-discharge care providers; document assessment, plan, interactions, and interventions per guidelines.
• Assist Registered Nurse Case Managers with discharge planning activities as requested; coordinate with RN Case Manager for significant community resources.
• Take, review, evaluate, and prioritize written and oral referrals; act as liaison with community agencies and resources; participate as lead in obtaining charity and financial resources, legal guardianship, adoptions, psychiatric referrals, and other needs.
• Other duties as assigned.

Qualifications

• Minimum Education Required: Master’s degree in Social Work
• Preferred: 3 years’ experience
