U.S. Department of Veterans Affairs is hiring: Social Worker in Indianapolis

The incumbent functions as one of the HUD-VASH Social Workers for the Medical Center's Health Care for Homeless Veterans Program (HCHV), HUD-VASH Program. The HUD-VASH program is a collaborative effort supported through HUD Section 8 rental assistance vouchers and VA's provision of intensive case management services. The primary goal of HUD-VASH is to move Veterans and their families out of chronic homelessness.

Duties

• Provide case management and support services to homeless Veterans and their families.
• Coordinate with community agencies to facilitate housing placement and support services.
• Assess the needs of Veterans and develop individualized care plans.
• Document all case management activities and progress accurately.
