- Participates in discharge planning, development and implementation of social care plans and resident assessments. Performs administrative duties such as completing medical forms, reports, evaluations, studies, charting, arranging transportation, etc., as necessary, coordinating activities with other departments.
- Participates in interdisciplinary team meetings.
- Promotes optimum transition of patient for appropriate and safe placement for discharge. Provides ongoing communication with patient and responsible persons regarding progress and status during stay.
- Provides information to the patient/family on Medicare/Medicaid and other financial assistance programs. Offers consultation to staff to address patient/family concerns related to psychosocial issues, discharge planning, and placement after SNU/ARU.
- Demonstrates understanding of rules and regulations governing SNU and ARU.
- Demonstrates autonomy, professional judgment, responsibility, and reliability.
- Provides written/oral reports on social service programs and activities as directed. Conducts grievance tracking and reports in appropriate meetings. Evaluates and implements recommendations related to social services.
- Participates in quality programs for SNU/ARU.
- Demonstrates flexibility to meet patient and family needs.
- Maintains confidentiality and complies with HIPAA regulations.
- Provides competent patient care by interpreting data, making decisions based on age-specific growth and development, and adapting procedures accordingly.
- Engages in personal and professional growth through participation in educational programs.
- Communicates and performs duties aligned with MercyOne North Iowa Medical Center's mission, values, and impact.
- Supports safety policies and proactively addresses safety concerns.
- Performs other duties as assigned by supervisors.

- Understands and adheres to rules governing SNU and ARU.

- Prepares reports and manages grievances related to social services.
- Engages in quality improvement initiatives.
- Adapts to meet patient needs and maintains confidentiality.
- Delivers patient care tailored to age and development needs.
- Participates in ongoing education and training.

- Identifies safety issues proactively.
- Performs additional duties as needed.
Minimum Qualifications:
- Must hold LMSW with recent hospital experience within the last year.
- Experience as an LMSW in an acute medical inpatient setting (not behavioral health).
- LMSW or LCSW certification.
