Job Summary:
Under minimal supervision and at the direction of the Case Management Director, the Inpatient Social Worker/Patient Counselor helps patients address bio-psychosocial needs and navigate medical care and community resources. This role provides crisis intervention, short-term counseling, and supports patient transitions along the continuum of care. The Social Worker/Patient Counselor also coordinates discharge planning in collaboration with patients, families, and the healthcare team.

Required Qualifications:
- Master's Degree in Social Work or Counseling Psychology.
- One (1) year of experience as a Social Worker or Counselor.
- Two (2) years of experience in a hospital or acute care setting.
