Job Summary: Under minimal supervision and at the direction of the Case Management Director, the Inpatient Social Worker/Patient Counselor supports patients in addressing their bio-psychosocial needs, facilitating access to medical care and community resources to optimize their health outcomes. This role involves crisis intervention, short-term counseling, and transition planning, ensuring patients move smoothly along the continuum of care. The Social Worker/Patient Counselor also coordinates discharge planning by collaborating with patients, families, and healthcare teams.

Key Responsibilities:

• Support patients transitioning across different levels of care.

Required Qualifications:

• Master's Degree in Social Work or Counseling Psychology.
• Minimum one year of experience as a Social Worker or Counselor.

Preferred Qualifications:

• Two years of experience in a hospital setting.
