Community Support Specialist Job at Circle of Care St Louis in Saint Louis

At Circle of Care, St. Louis, we’re passionate about supporting individuals and families, and we’re seeking a dedicated Community Support Specialist (Hybrid) to join our team. This role combines office-based support with field-based caregiving and outreach.

• Conduct intake interviews, home visits, welcome visits, and case management visits in the community.

• Support individuals transitioning from Skilled Nursing Facilities (SNFs).

• Assist with community integration and benefits navigation.

• Provide personal care support (ADLs, respite, APC tasks if authorized).

• Recruit new clients and attend community events and resource fairs.

• Complete accurate documentation in CareSmartz360 and Charity Tracker.

• At least 1 year of experience in social services, healthcare, or a related field.

• Compassionate, organized, and an excellent communicator.

• Able to work a flexible schedule (includes evenings/weekends).

• Able to lift at least 35 lbs to assist clients.
