$45,000 - $47,000 (non-negotiable)

Qualifications and Competencies
Bachelor’s Degree in Social Work, Psychology, or related field; Master’s Degree preferred.
Minimum of three (3) years relevant experience in case management or providing training, mentoring, support to disadvantaged populations, including at-risk and justice-involved youths and adults.
Experience in education and developing relationships with service providers.
Ability to build and maintain relationships and partnerships; maintain confidentiality and professionalism.
Knowledge of case management principles and Miami-Dade community systems.
Knowledge of motivational interviewing techniques.
Strong leadership, organizational, and analytical skills.
Ability to manage multiple tasks and meet deadlines.
Excellent interpersonal and communication skills.
Proficiency with Microsoft Office Suite and internet navigation.
Ability to coordinate with staff, agencies, and the public.
Other duties as assigned.
Additional Notes
Must have a valid Driver’s License and insurance coverage for personal vehicle use.
Physical ability to walk, sit, stand, reach, talk, hear, and lift materials.

Position Summary The Case Manager will handle Navigational Case Management for participants in Project ACTION, supporting their transition and reintegration through assessments, planning, coordination, and resource development.
Essential Duties
Conduct outreach, recruit participants, verify eligibility, and document according to guidelines.
Administer assessments and develop Individual Development Plans (IDP).
Monitor participant progress and provide guidance and mentorship.
Maintain case files and track referral outcomes.
Build relationships with participants and community organizations.
Prepare reports for funders and partners.
Represent Gang Alternative, Inc. professionally.
Implement navigational case management with excellence and compliance.
