Summary:
Achieve the physical, mental, and social well-being of the client within agency policy. Support utilization of medical resources through clinical expertise. Monitor clinical, functional, and financial outcomes. Assume responsibility for ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver. Communicate with the physician responsible for the home health plan of care and other healthcare practitioners as appropriate. Responsibilities
Assess physical, emotional, socio-economic, and environmental needs of patients and their families. Provide services ordered by the physician as indicated in the plan of care. Communicate with all physicians involved and other healthcare practitioners as appropriate. Develop, implement, and evaluate the plan of care in partnership with the patient, caregiver, and physician(s). Coordinate services with the patient, family, and others involved. Provide counseling and education to patients and caregivers. Regularly evaluate patient progress, respond promptly to changes in condition, and terminate care when goals are met or as per agency policy. Participate in quality assurance, performance improvement programs, and agency-sponsored training. Identify additional home health service needs and refer patients when appropriate. Supervise services rendered by homemaker/home health aides according to guidelines, and submit periodic evaluations. Fulfill all compliance responsibilities, including preparing clinical notes and attending training. Perform other duties as assigned. Education
Associate's degree or equivalent; RN diploma with over 3 years of related experience considered in lieu of degree. Other Requirements
Strong research skills, clear communication, knowledge of systems integration. Minimum 1 year experience; Bachelor's Degree preferred. Experience in a qualified home health agency is a plus. Working Conditions
Manual skills and motor coordination required. Little or no occupational risk. Generally pleasant working environment. Physical Effort
Standard physical environment. Credentials
