Position Duties Discharge planning, MDR, Referrals, Working with Family and Patients.

Minimum Required Qualifications LCSW or LMSW (must be licensed in FL); BLS; 2 years acute care adult hospital case management; Knowledge of Hospital DC planning/execution and knowledge of MDR or IDT; MIDAS/Meditech, PASRR, and Navihealth experience.

Length of Assignment 13 weeks

Shift / Hours Per Week 8:00AM - 4:30PM with every third weekend
