HOACNY provides cancer care services, Monday through Friday, day hours at three convenient locations in East Syracuse, Syracuse and Auburn. This includes newly diagnosed breast cancer, high risk patients and patients with benign breast disease. Provide support, education and guide newly identified breast cancer patients or those identified to be at high risk through the health care system.Assist patients in understanding testing, diagnosis and treatment options as presented by surgeons, physicians, and other health care providers. Develop and/or maintain learning library for patient educationCare Coordination and ReferralsFacilitate referrals of patients to needed services including: imaging, surgery, medical oncology, radiation oncology, social work, dietician, mentor program, wellness center, survivorship program, genetic counseling, cancer rehabilitation and other support agencies and systems as is appropriate for patient needs.Identify barriers to care and facilitate removal of barriers to care for patients. Facilitate interaction with various health care professionals and their staff to improve access and coordination of care for patients.Participate in interdisciplinary Breast Cancer Tumor board. Assist in preparation of breast cancer tumor board planning and documentation as requested. Provide help as needed in obtaining insurance and/or forms required for financial and other types of assistance. Provide clinical rational for advanced imaging as requested.Develop and maintain professional relationships with health care providers in the community. Quality Measurement and ImprovementTrack interventions and outcomes as required Participate in quality measurement per accredited program requirements Understand accreditation standards and work toward goal of Breast Program AccreditationCommunity Outreach and EducationBuild relationships with other patient navigators. health care providers and the general public Evaluate whether or not the navigation program is meeting its intended purpose and strategies to improve patient navigation and breast care.Implement strategies to promote awareness of the breast navigation program and Breast Care Partners in the organization and the community Serve as a community resource for breast disease, screening and prevention programs.Hematology Oncology Associates of CNY is committed to creating a diverse environment and is proud to be an equal opportunity employer. The Breast cancer navigator must have outstanding interpersonal, oral and written communication, organizational and multi-tasking skills. The individual must poses knowledge of breast cancer, breast cancer treatment modalities, community resources and healthcare structure and function. Salary Description $30.00+
