Breast Program Patient Navigator-Registered Nurse - Monday through Friday (no nights, weekends or holidays)
Breast Care Partners is an NAPBC accredited program providing an organized, comprehensive, multidisciplinary, team approach to breast care. We are a collaboration of dedicated breast care specialists from Hematology Oncology Associates of CNY, Crouse Medical Practice Surgical and Bariatrics, Crouse Health, CRA Medical Imaging, St. Joseph's Physicians Surgical Services, St. Joseph's Health, and St. Joseph's Imaging Associates. Together we are committed to improving breast patient outcomes in our community.
The Breast Care Partners Nurse Navigator provides patient care coordination by serving as a liaison between the patient and the practitioners and services she may require. The Nurse Navigator acts as a patient advocate, provides patient and family education, and coordinates all aspects of care to assure the provision of seamless comprehensive services. The Nurse Navigator plays a critical role in the patient experience including community outreach, optimizing patient services and working collaboratively with the team to create a patient centered environment. This involves developing expertise in the operations and systems of Breast Care Partners to promote and maintain health and wellbeing in women. The scope of navigation extends beyond the services provided within Crouse Medical Practice Surgical Center and is intended to coordinate care across the continuum in a comprehensive way. The role of the Nurse Navigator is an integral part of Breast Care Partners commitment to provide holistic patient centered care for women.
You Will Be Responsible For :
Clinical Practice: Identifies new patients during pre-diagnosis journey and offers navigation services from the beginning of the patient's diagnosis journey to survivorship or end of life.
Establishes communication with physicians, physician extenders and their office staff to outline navigation programs and available resources. Collaborates with the primary medical oncologist, radiation oncologist and/ or oncology surgeon, to act as a resource for the patient and family beginning during initial consultation and continuing through treatment.
• Assesses physical, psychological and educational needs of patients according to guidelines, standards of care and individual needs.
• Assesses patient care needs and/or changes in condition throughout treatments and procedures and communicates findings to the provider.
• Coordinates plan of care for patients, including, but not limited to, appointments, transportation, education, and follow up treatments to ensure timely delivery of diagnostic and treatment services.
• Coordinates referrals to medical and radiation oncology, physical therapy, and other services as determined by providers. Builds and maintains professional relationships with the medical oncologists, radiation oncologists, surgeons, other medical physicians and professionals involved in the patients care.
• Provide and coordinate education and information to patient and family to ensure a seamless, continuous, and comprehensive continuum of care.
Prepares for, and Participates in, the Tumor Board as needed.
Develops and participates in performance improvement activities and development of guidelines in collaboration with Medical Directors and physician leaders to improve patient outcomes, assures appropriate utilization of resources and increases patient satisfaction.
Ensures all requirements of regulatory standards and accreditation, for patient care are met.
Other duties and/or projects as assigned.

Requirements
Basic Knowledge, Skills and Education Requirements : The ideal candidate should be a registered nurse with 3 or more years of experience in medical surgical nursing and oncology. The Breast cancer navigator must have outstanding interpersonal, oral and written communication, organizational and multi-tasking skills. The individual must poses knowledge of breast cancer, breast cancer treatment modalities, community resources and healthcare structure and function.
