Hospice Aide
PRN Position Job Address: 230 Welcome Way Blvd West Bldg 4 Suite A15 Indianapolis, IN 46214 Job Description Summary
Responsible for providing support services, under the supervision of the appropriate professional staff, that assists the patient or family in the achievement of physical and emotional comfort. Provides personal care and activities of daily living assistance as needed. Essential Job Functions/Responsibilities: Provides personal care and activities of daily living per plan of care. Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of patients, transferring patients, assisting with the normal range of motion and positioning, and completing limited household chores. Prepares and provides adequate nutrition and fluid intake. Provides medically prescribed diets under supervision. Observes and reports changes in patient condition or expectations of service promptly to a skilled nurse via telephone and documents communication in clinical note in the Electronic Medical Record (EMR) Recognizes emergency situations, and implements appropriate emergency procedure when indicated. Demonstrates utilization of good basic infection control procedures. Follow agency procedures especially with regard to infection control, handling of hazardous wastes, and safety measures. Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences. Accurately documents care provided and submits visit note within 24 hours of visit. Attends a minimum of twelve (12) hours of in-service programs annually according to agency policy. Attends patient care conferences as directed. Renders services in strict accordance with the written care plan as assigned to the hospice aide by the RN. Ensures compliance with state and federal regulations, company policies/procedures and process maps in the performance of job responsibilities Maintain patient, family and employee confidentiality. Promote teamwork, accountability and positive attitudes. Contribute to team effort by accomplishing related results, as needed. Adheres to the Organizations documentation and care procedures and standards of personal and professional conduct Position Qualifications Possesses a STNA approved by an accredited organization, within the state. High school diploma or General Education Degree (GED) required. Two years of appropriate experience in hospice preferred. Demonstrates good verbal and written communication and organization skills. Possesses and maintains current CPR certification. Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and is in good working order.
