Serve as primary point of communication
for residents and their families regarding Plans of Care, therapies, treatment protocols, and ongoing care updates to ensure understanding and trust. Collaborate with interdisciplinary teams
by participating in daily stand-up meetings and coordinating care with providers, financial services, and third-party payers. Monitor and respond to changes
in residents' conditions, adjusting care plans as needed and performing utilization reviews to ensure timely and cost-effective treatment. Manage admissions, discharges, and referrals , coordinating both internal and external transitions of care for optimal patient outcomes. Maintain accurate, timely documentation
and charting in compliance with policies, procedures, and regulatory requirements. Educate residents and families
about diagnoses, treatment options, and care progressions to promote informed decision-making and reduce anxiety. Demonstrate strong interpersonal skills , maintaining professional, respectful communication with residents, families, staff, and external partners. Apply sound judgment and independent decision-making
in complex care situations involving multiple stakeholders. Possess working knowledge of Medicare, Medi-Cal, and Medicaid
programs, as well as experience navigating PCC (PointClickCare) systems. Meet licensing and credentialing requirements , holding an active LVN, LPN, or RN license (RN preferred), with CCM certification considered a plus.
Pay Range: $55-60/hr
Shift: PM (3pm - 11pm)
