Responsibilities

  • Assess the psychosocial status of patients and families/caregivers related to terminal illness and home environment, and communicate findings to the interdisciplinary care team.
  • Conduct social evaluations including family dynamics, caregiver abilities, communication patterns, suicide risk, neglect, or abuse, and plan interventions.
  • Counsel patients and families in relation to stress and coping difficulties; provide crisis intervention when necessary.
  • Provide ongoing bereavement support to individuals identified as moderate or high-risk; assist in planning funeral arrangements and related responsibilities.
  • Educate patients and families on advance directives; provide information and referrals for practical and environmental needs; liaise with community agencies.
  • Maintain and develop contracts with public and private agencies as resources for patient care and staff.
  • Participate in the development of the individualized plan of care, attend interdisciplinary meetings, and support the team in recognizing psychosocial stresses on symptoms of terminal illness.
  • Assist the physician and team in understanding social and emotional factors related to health problems and death/dying.
  • Support bereavement leadership to meet the needs of families/caregivers for up to 13 months following death, ensuring regulatory compliance.
  • Ensure HIPAA compliance and maintain patient information as confidential, using a minimum necessary approach.

Qualifications

Requirements/Qualifications

  • Bachelor’s degree in Social Work from an accredited institution.
  • Current licensure in the state(s) of practice with one of the following licensure levels: CSW, CAPSW, CISW, LCSW, SWTC. Social workers may hold compact licenses where applicable.
  • Ability to pass background check, consumer report, and drug test.
  • Valid State Driver’s License and access to a properly insured vehicle; acceptable driving record required.

Preferred

  • Master’s degree in Social Work.
  • 1+ year of social work experience in a health care setting.

Physical Requirements

  • Physical requirements to perform essential duties; reasonable accommodations may be provided.
  • Pushing/pulling and lifting/carrying up to 12 pounds, occasionally up to 20 pounds.
  • Ability to adapt to various work conditions in a home setting to ensure high-quality patient care.
