SUMMARY

Family Service League is seeking a

full-time

Supervisor for the Health Homes Bay Shore program. The Supervisor will supervise and support the staff to ensure compliance with OMH and DOH guidelines. The Supervisor will oversee the daily operations of the program.

RESPONSIBILITIES

The Supervisor will oversee staff, including recruiting, hiring, training, developing, and conducting supervisions and performance appraisals. Conduct chart reviews. Assist with adherence to program standards. Interface with adult SPOA, DOH and Health Homes Adult Care Management. The Supervisor will assist with interviewing, hiring, evaluating and with training of staff. Review time records, schedules and approves vacation/time off requests. Attend program meetings in absence of the Program Director. All other duties as assigned.

QUALIFICATIONS

Bachelor's degree in a health-related field is required. Master's degree preferred.

Minimum of 4 years of experience working with youth and/or adults diagnosed with mental and/or physical illness required.

At least 1 year of previous supervisory experience required.

Knowledge of community resources and experience with or strong familiarity with Case Management required.

Excellent clinical, leadership, and organizational skills.

Strong communication skills, both verbal and written.

Proficient computer skills, including Microsoft Office required.

Valid and clean NYS Driver's License required.

PHYSICAL REQUIREMENTS

This position often requires sitting and working at a computer for extended periods of time, traveling to different FSL offices, community meetings, home visits, etc.
