Job Title: School Social Worker - LEAP Program
Location: Tolland Intermediate School and Tolland Middle School
Program: Learners Excelling and Progressing (LEAP)
Reports To: Pupil Services Administrator

Position Overview:
We are seeking a compassionate and skilled School Social Worker to join our team. The School Social Worker for the LEAP program will provide specialized social-emotional and behavioral support to students with identified disabilities. This role focuses on helping students overcome barriers to learning by addressing their social, emotional, and behavioral needs within a specialized educational setting. The social worker will collaborate with educators, families, and community resources to promote student success across Tolland's district-wide LEAP program.

Key Responsibilities:
  • Provide direct social work services, including counseling and crisis intervention, to students identified through MTSS (Multi-Tiered System of Supports) or other behavioral referral processes
  • Lead or co-lead small group and classroom-based sessions focused on emotional regulation, conflict resolution, and pro-social skills
  • Conduct assessments to identify student needs and develop individualized support plans.
  • Collaborate with teachers, administrators, and special education staff to create and implement strategies that support students' access to the curriculum.
  • Serve on MTSS team, crisis intervention teams, and support de-escalation strategies
  • Support positive behavioral interventions and facilitate social skills development.
  • Serve as a liaison between the school, families, and community agencies to coordinate services and resources.
  • Participate in Planning and Placement Team meetings, offering social-emotional and behavioral insights and recommendations.
  • Maintain accurate documentation and comply with all district policies and procedures.
Qualifications:
  • Master's degree in Social Work (MSW) from an accredited program, LICSW preferred.
  • School Social Worker Certification
  • Experience working with children and adolescents with social-emotional and behavioral disabilities preferred.
  • Strong knowledge of special education laws, behavioral interventions, and counseling techniques.
  • Excellent communication, collaboration, and organizational skills.

Population Served:
Students within the LEAP program at Tolland Intermediate, Middle, and High Schools who have social-emotional and/or behavioral disabilities that significantly impact their educational progress.
