Social Worker – Complex Care Team Location: Westminster, UK Salary: £32 per Hour (Full-time, Locum) Duration: 3 Months

What you will do:

• Conduct comprehensive Care Act assessments and develop detailed care plans.
• Execute risk assessments, reviews, and manage service referrals, ensuring the needs of each individual are met efficiently.
• Lead safeguarding enquiries as a Lead Enquiry Officer, liaising closely with the Safeguarding Operational Team.
• Undertake duty work including occasional on-the-ground duty visits and casework for those in urgent circumstances.
• Collaborate closely with a multi-disciplinary team (MDT) and engage in regular team meetings to discuss case progression and strategies.
• Utilize advanced software such as MOSAIC or FrameworkI (FWI) for care management and create thorough reports using MS Office applications.
• Support individuals in achieving greater independence through strength-based work approaches.
