This is a remote position with the flexibility to conduct at-home visits in the Las Vegas area. You'll be a key part of our team, helping members and their families navigate their health journey and find the resources they need to thrive.
About the Role:
In this role, you'll assess and evaluate members' needs, guiding them toward the right resources for their care and well-being. You'll work closely with a multi-disciplinary team, using your skills in individual and family counseling, crisis intervention, and case management to support members with their physical, environmental, and psycho-social health issues.
Key responsibilities include:
- Conducting telephonic outreach to assess members' health and well-being.
- Providing at-home assessments and developing individual care plans for high-risk members.
- Collaborating with physicians and other healthcare professionals to create and implement comprehensive care plans.
- Conducting psychosocial assessments and providing counseling to help patients and families deal with chronic and acute illnesses.
- Developing and maintaining relationships with community resources to support members.
- Integrating social work and nurse case management as a team.
Who We're Looking For:
- Education: A Master's Degree in Social Work (MSW) is required. We are open to candidates with their LMSW, LISW, or LCSW.
- Skills: You are resourceful, quick on your feet, and have a thick skin.
Details:
- Salary: $90,041.60 per year
- Location: Remote with some at-home visits in the Las Vegas area
- Schedule: Monday to Friday, 8 am - 5 pm, with some flexibility.
