Nurse Practitioner/Primary Care Provider
Salary Range:
$181,749 - $254,499
Position Overview As a Nurse Practitioner, you'll play a key role in delivering compassionate, comprehensive care to our senior participants. Youll perform in-person assessments, collaborate with our interdisciplinary team (IDT), and help shape high-quality, coordinated care plans. Key Responsibilities
Conduct in-person, comprehensive health assessmentsincluding initial, semi-annual, unscheduled, and ongoing evaluations Partner with physicians and the IDT to develop and adjust individualized care plans Participate in discharge planning from both acute and long-term care settings Provide continued care for participants residing in nursing homes through regular, needs-based visits Coordinate closely with internal and community-based providers to ensure seamless, effective care Support quality improvement efforts by reviewing outcomes, hospitalization data, and specialist utilization at the direction of the Medical Director Participate in a rotating on-call schedule for nights and weekends Qualifications
Graduate of an accredited nursing program Certified Nurse Practitioner with active Prescriptive Authority Licensed as a Nurse Practitioner in the state Minimum of two years experience working with geriatric or senior populations Strong understanding of chronic care and geriatric best practices Valid state drivers license and a clean DMV record Proof of current auto insurance What We Offer Join a purpose-driven team where your voice matters.
